When the numbers stop lining up.

A hospital record can be accurate within its own reporting boundary and still mislead a reader who assumes it describes one campus, one year, or one bedside workforce. These checks expose where closer investigation is required.

7reviewed bed-scope mismatches
7staffed counts above licensed counts
35contract shares at or above 15%
34facilities without identified RN hours

Counts are calculated directly from the Observatory’s 444-facility dataset and its documented evidence reviews. These are descriptive review flags, not safety scores, regulatory findings, or statewide prevalence estimates.

A campus is not always the reporting unit.

COMMUNITY REGIONAL MEDICAL CENTER - FRESNO

Reporting dataset: 1075 licensed beds. Official current campus profile: 685 licensed beds.

The 1,075-bed reporting denominator is license-wide, not the 685-bed Fresno campus alone. The additional 390 beds belong to the broader license scope; their campus-by-campus allocation has not been independently reconciled.

HOAG MEMORIAL HOSPITAL PRESBYTERIAN

Reporting dataset: 536 licensed beds. Official current campus profile: 440 licensed beds.

Workforce and capacity values represent the combined 536-bed reporting scope, not the 440-bed Newport Beach campus alone.

SHARP MEMORIAL HOSPITAL

Reporting dataset: 862 licensed beds. Official current campus profile: 656 licensed beds.

Workforce and capacity values represent the combined 862-bed reporting scope, not Sharp Memorial’s 656-bed campus alone.

PALOMAR MEDICAL CENTER

Reporting dataset: 286 licensed beds. Official current campus profile: 306 licensed beds.

Both bed figures appear in official California records, but their reporting dates and the effective date of the 20-bed change have not been reconciled. Historical contract reliance remains unknown.

KAISER FOUNDATION HOSPITAL - MANTECA

Reporting dataset: 237 licensed beds. Official current campus profile: 61 licensed beds.

The 237-bed reporting-period dataset figure combines Manteca and Modesto: 61 parent-facility beds plus 176 consolidated Modesto beds. Workforce and financial measures describe the combined reporting scope; neither campus has a separately established payer or staffing value.

DOCTORS MEDICAL CENTER - MODESTO

Reporting dataset: 461 licensed beds. Official current campus profile: 394 licensed beds.

The 461-bed dataset figure spans the parent and separately listed behavioral-health facility. It must not be presented as the main hospital campus bed count.

OAK VALLEY HOSPITAL DISTRICT

Reporting dataset: 144 licensed beds. Official current campus profile: 29 licensed beds.

The 144-bed reporting-period total combines acute hospital and nursing-facility scopes. Do not describe it as 144 acute-care hospital beds or compare its 48.4-day aggregate average stay directly with general acute-care peers.

A difference can reflect consolidated facilities, a broader license boundary, or different effective dates. The dataset value is not automatically wrong; the campus-level interpretation may be.
